Coordinated Entry Administrator

Help lead the community’s response to homelessness. Make a lasting impact in Addison County.

Charter House Coalition is seeking a dedicated, organized, and compassionate Coordinated Entry Administrator (CEA) to oversee Addison County’s Coordinated Entry system and help ensure individuals and families experiencing homelessness are connected to the housing and services they need.

This position is ideal for someone who enjoys balancing meaningful client interactions with data management, community collaboration, and system coordination. As the Lead Agency Administrator, you’ll play a vital role in improving access to housing while supporting community partners and ensuring compliance with HUD and Housing Opportunity Program (HOP) standards.

Position Details

• Full-Time, in-person: 40 hours per week
• Schedule: Monday–Friday, 8:00 AM – 4:00 PM
• Pay Range: $28.00–$30.00 per hour, based on experience

Primary Responsibilities:

• Manage Addison County’s Coordinated Entry system in accordance with HUD, HMIS, and Housing Opportunity Program (HOP) standards.
• Conduct Coordinated Entry assessments and shelter intakes.
• Coordinate housing referrals and service connections for individuals and families experiencing homelessness.
• Work closely with housing providers, service agencies, and community partners to ensure equitable access to housing resources.
• Maintain the accuracy, integrity, and quality of HMIS and Coordinated Entry data.
• Monitor system performance, complete required reporting, and ensure compliance with federal, state, and local requirements.
• Facilitate weekly Housing Solutions meetings and collaborate with community stakeholders.
• Provide training and technical support to staff and partner agencies on Coordinated Entry and HMIS procedures.
• Assist with the annual Point-in-Time (PIT) Count and other community-wide housing initiatives.
• Support shelter diversion, housing stabilization, and limited housing navigation as needed.

Qualifications:

• Excellent communication and organizational skills.
• Experience with case management, housing services, homeless response systems, HMIS, or social service programs.
• Strong analytical skills and attention to detail.
• Experience maintaining accurate data and preparing reports.
• Knowledge of Housing First, trauma-informed care, crisis intervention, and person-centered service delivery.
• The ability to build collaborative relationships with community partners and service providers.
• Strong computer skills and experience using electronic documentation systems.
• The ability to work independently while contributing to a collaborative team.

Full-Time/Part-Time Housing Case Manager

Charter House is seeking a compassionate, motivated, and organized Housing Case Manager to join our team. If you’re passionate about supporting individuals experiencing homelessness and want to make a meaningful impact in your community every day, we’d love to hear from you.

Position Details

• Full-Time, in-person: 40 hours per week/10 hours per week
• Schedule: Monday–Friday
• Flexible Hours: Work hours can often be adjusted to accommodate individual and client needs while meeting program requirements.
• Pay: $25-26 per hour, based on experience

Primary Responsibilities

As a Housing Case Manager, you will provide individualized support to people experiencing or at risk of homelessness by:

• Completing shelter intakes and comprehensive assessments.
• Developing housing stability plans with each participant.
• Connecting guests with housing, healthcare, mental health, substance use treatment, employment, education, childcare, legal services, and public benefits.
• Assisting with housing searches, rental applications, landlord communication, lease education, and move-in coordination.
• Advocating for clients by attending appointments and collaborating with community partners.
• Maintaining accurate case notes and documentation.
• Managing a caseload of approximately 12–18 individuals, meeting with each participant weekly or more frequently as needed.
• Participating in Coordinated Entry, housing coalition meetings, staff meetings, and ongoing professional development.
• Supporting shelter operations and participating in an on-call rotation every 3–4 weeks.

• Compassionate and committed to serving people experiencing homelessness.
• Skilled at building trusting relationships with individuals from diverse backgrounds.
• Experienced in case management, social services, peer support, crisis intervention, or a related field.
• Familiar with trauma-informed, person-centered care.
• Organized, dependable, and comfortable using electronic documentation systems.
• Able to work independently while collaborating effectively with a team.
